Swelling after lip filler is normal, predictable, and temporary. It peaks in the first 24 to 48 hours and settles over two weeks. Most patients are comfortable in public by day four or five. This blog covers the day-by-day swelling timeline, factors that affect swelling, what the red flags are, and when the final result is actually visible.

Two weeks is the honest answer. Most of the visible swelling resolves in the first four to five days, but the lip does not fully settle into its final shape until around the two-week mark. What patients see immediately after treatment is not the result. It is the result plus swelling, plus the initial water uptake from hyaluronic acid, plus the body’s response to the needle. All of that resolves. What remains at two weeks is what was actually injected.

According to Dr Harikiran Chekuri, one of India’s pioneering plastic surgeons, “The most common call I get after lip filler is on day two, when swelling peaks and the patient is convinced something has gone wrong. In most cases, it hasn’t. The timeline is predictable. Knowing it in advance stops patients from making decisions based on what they see at 48 hours.”

What Does the Swelling Timeline Actually Look Like?

The pattern is consistent across patients. Volume of filler, individual sensitivity, and injector technique affect the degree, not the sequence.

Timeframe

What to Expect

Immediately after

Lips appear significantly larger. Mild numbness from lidocaine in the filler. Firm to touch

Hours 12 to 36

Peak swelling window. Lips at their largest. Bruising may appear. Normal

Day 2 to 3

Swelling still significant. Bruising darkens before it fades. Most patients stay home these days

Day 4 to 5

Swelling noticeably reduces. Most patients comfortable in public

Day 7

Lips look close to final result. Minor residual swelling still present

Week 2

Filler fully integrated with tissue. Swelling resolved. Final shape visible

Week 2 to 4

Any minor lumps or asymmetry from the healing process resolve in this window

  • Why swelling happens: Hyaluronic acid is hydrophilic. It attracts water, which is what creates volume, but it also causes initial puffiness as the product draws moisture into the tissue. The needle itself creates minor trauma that the body responds to with inflammation. Both are normal and both resolve
  • Why day two looks worse than day one: Swelling and bruising peak between 12 and 36 hours post-treatment. Patients who look in the mirror on day two and see larger, more bruised lips than they had immediately after treatment are seeing normal inflammatory progression, not a complication
  • Volume of filler affects degree not duration: A full syringe causes more pronounced swelling than 0.5ml. The timeline is the same. The magnitude differs. First-time patients who start with a conservative volume have a gentler swelling experience, which is one clinical reason to start lower
  • Do not assess the result before two weeks: Asymmetry, lumps, and uneven fullness at day five frequently resolve on their own as swelling distributes and the filler integrates. Patients considering Dermal Fillers at Redefine are counselled on this timeline at every consultation so the two-week window is not a surprise

What Is Not Normal and When to Call Your Injector?

Most of what patients worry about in the first two weeks is normal. A small number of signs are not.

  • Increasing pain after day two: Swelling and tenderness reduce after 48 hours. Pain that increases rather than decreases after that point is not part of normal healing
  • Skin colour change around the lips: White, blue, or mottled skin in the area surrounding the injection site is a sign of possible vascular occlusion, a rare but serious complication where filler compresses or enters a blood vessel. This requires immediate contact with the injector, not a wait and see approach
  • Hardness that does not soften: Some firmness in the first week is normal as hyaluronic acid holds water. A lump that remains hard and defined at two weeks and beyond warrants assessment
  • Fever or spreading redness: Signs of infection. Not swelling. Contact your provider
  • Asymmetry at two weeks: Minor asymmetry during the swelling phase is normal. Asymmetry that persists at two weeks when swelling has resolved is a different matter and should be assessed at a follow-up appointment
  • Patients considering lip filler who want to understand how it compares to a surgical option can read Lip Lift or Lip Filler: Which Is Better? at Redefine, which covers what each treatment actually corrects and which concern each one is suited to

Frequently Asked Questions

How long does lip filler swelling last?

Most significant swelling resolves within four to five days. Residual swelling and the final settling of the filler takes up to two weeks. The result visible at two weeks is the actual outcome.

When does lip filler swelling peak?

Between 12 and 36 hours after treatment. Day two is typically when lips appear largest and most bruised. This is normal inflammatory progression, not a complication.

Can I go to work after lip filler?

Most patients return to desk-based work the next day. Days two and three are when swelling is most visible. Patients with public-facing roles often plan treatment before a weekend or days off.

Why do my lips look uneven after filler?

Swelling distributes unevenly during healing. Asymmetry in the first two weeks is common and usually resolves as swelling subsides and the filler integrates. If asymmetry persists at two weeks, a follow-up assessment is appropriate.

When should I contact my injector after lip filler?

Immediately if you notice skin colour change around the lips, increasing pain after day two, spreading redness, or fever. These are not swelling. They are signs that require prompt assessment.
