A neck lift fixes the neck. A facelift fixes the mid and lower face. They overlap at the jawline, and many patients need both. This blog breaks down exactly what each procedure corrects, where they work together, and what determines which one is right for you.
Different procedures. Different problems. Significant overlap at the jawline.
A neck lift and a facelift are not interchangeable. Each targets a specific anatomical zone. The confusion comes from the fact that they share an incision site and are frequently combined, which makes them sound like the same operation. They are not. Knowing what each one corrects is the only way to understand which one you need.
According to Dr Harikiran Chekuri, one of India’s pioneering plastic surgeons, “The most common mistake patients make is assuming a facelift fixes the neck or a neck lift fixes the face. Each does its job well within its own zone. When both zones are affected, which is common, the right answer is usually both procedures done together, not one instead of the other.”
What Does a Neck Lift Fix?
Everything from the chin to the collarbone. Nothing above the jawline.
A neck lift addresses the specific changes that occur in the neck with age: loose skin, vertical muscle bands, fat accumulation under the chin, and loss of the jaw-to-neck angle definition. It is the right procedure when the primary concern is the neck, not the face.
- Platysmal bands: The vertical cords that appear on the neck with age are caused by the platysma muscle separating and pulling forward. A neck lift tightens and repositions this muscle through a small incision under the chin. This is the only procedure that corrects platysmal banding
- Loose neck skin: Excess skin along the neck and below the chin is removed and the remaining skin is redraped tightly. This restores the smooth neck contour that disappears with age and significant weight loss
- Double chin from fat: Submental fat under the chin is removed either by liposuction or direct excision during neck lift. In patients where fat is the primary concern and skin laxity is minimal, liposuction alone may be sufficient
- Loss of jaw-to-neck angle: The sharp definition between the jawline and neck softens with age. A neck lift restores that angle by tightening the underlying muscle and removing excess skin and fat in the submental region
- What it does not fix: A neck lift does not address jowls, nasolabial folds, cheek descent, or any concern above the jawline. If the lower face has also aged, a neck lift alone will leave those areas unchanged
For patients in Hyderabad, facial surgery at Redefine covers the full range of surgical facial rejuvenation procedures under the supervision of Dr Harikiran Chekuri.
What Does a Facelift Fix?
The mid and lower face. Not the neck on its own.
A facelift repositions the deeper facial tissues, removes excess skin, and tightens the SMAS layer, the structural layer beneath the skin that descends with age. It addresses the face from the cheekbones down to the jawline.
- Jowls: The sagging tissue that forms along the lower jaw with age is one of the clearest signs that a facelift is indicated. Fillers and thread lifts can delay this, but once jowls are established, a facelift surgery is the only procedure that corrects them structurally
- Nasolabial folds: Deep lines running from the nose to the corners of the mouth deepen as the cheek fat descends. A facelift repositions the underlying tissue, reducing these folds without simply filling them
- Cheek descent: The mid-face drops with age, flattening the cheeks and creating hollows under the eyes. SMAS repositioning during a facelift restores the cheek position upward and back to where it sat in earlier years
- Marionette lines: The lines that run from the corners of the mouth downward are addressed by lifting the tissue above them. A facelift reduces marionette lines by repositioning the underlying tissue rather than adding volume to the line itself
- What it does not fix: A standard facelift does not address platysmal banding or significant loose neck skin below the jawline. Patients with both facial and neck aging typically need a facelift and neck lift performed together.
Patients wondering whether non-surgical options are still relevant can read What Works for Non-Surgical Jawline Sculpting? which covers when non-surgical treatments are appropriate and when surgery is the clearer answer.
The right procedure starts with knowing which zone is causing the problem. The two procedures are frequently the answer together, not separately.

Frequently Asked Questions
What is the difference between a neck lift and a facelift?
A neck lift addresses the neck, including loose skin, platysmal bands, and submental fat. A facelift addresses the mid and lower face, including jowls, cheek descent, and nasolabial folds. They share an incision site and are often combined.
Can a facelift fix the neck?
Partially. A facelift tightens the upper neck and jawline as part of the SMAS repositioning. It does not correct platysmal banding or significant loose neck skin below the jawline. A neck lift is required for those concerns.
Can a neck lift fix jowls?
No. Jowls are a facial concern, not a neck concern. A neck lift does not reposition the facial tissue above the jawline. A facelift is required to correct jowls.
Do most patients need both a facelift and a neck lift?
Many do, because the face and neck age together. The right combination is confirmed at consultation after assessing both zones individually.
How long do facelift and neck lift results last?
Results typically last 8 to 12 years depending on skin quality, lifestyle, and sun exposure. Non-surgical maintenance with fillers, Botox, and skin tightening extends the result.
