FUE Hair Transplant for Stage 5 Baldness

At 27, he had already lost what most men do not lose until their forties. A complete frontal and midscalp recession, classified as Norwood Hamilton Stage 5. He had spent years watching it progress, tried everything available without a prescription, and finally walked into Redefine Clinic knowing he needed something definitive. Dr Harikiran Chekuri performed a single-session FUE hair transplant with 4800 grafts. The procedure was completed without complications.

Post-Operative Care Instructions

Photographs document the full scope of the procedure: hairline design markings drawn prior to surgery, graft placement across the frontal zone and midscalp, and immediate post-operative appearance with protective dressings in place.

Fig. 1: Pre-operative hairline design markings, intraoperative graft placement, and immediate post-operative views. Frontal zone and midscalp covered in a single session. No complications recorded.

THE PROBLEM

He was 27, but the hairline told a different story. The entire frontal zone was gone. The midscalp had thinned to the point of transparency. What remained was the classic Stage 5 pattern: a rim of hair at the back and sides, an island of coverage at the vertex, and a wide expanse of bare scalp connecting them.

It had started in his early twenties, earlier than he expected and faster than he was prepared for. He tried topical treatments. They did not stop what was coming. By the time he came in for a consultation, the recession was extensive and stable enough to plan around.

For a 27-year-old, this degree of loss carries a particular weight. Every social interaction, every professional setting, every photograph is a reminder. He had not come in looking for a partial fix. He wanted his hair back, as completely as possible, in a single procedure.

CONSULTATION & TREATMENT PLAN

What Was Assessed During the Consultation

  • Norwood Hamilton Stage 5 confirmed: extensive frontal and midscalp baldness with intact occipital and temporal rim
  • Donor density: adequate for a high-volume single session
  • Hair characteristics: coarse caliber, dark, favourable for coverage
  • Scalp laxity: suitable for FUE extraction
  • Future loss trajectory discussed; medical therapy recommended post-procedure to protect native hair
  • Patient goals: maximum frontal and midscalp coverage, natural hairline, no visible donor scarring

Why FUE Was Chosen

  • No linear scar, preferred for patients who wear hair short
  • Allows high-volume graft harvesting across a wide donor zone
  • Precise individual extraction reduces trauma to surrounding follicles
  • Suitable for staged sessions if additional coverage is needed later
  • Faster return to normal activity compared to strip method
  • Donor area remains usable for future procedures if required

Surgical Approach

    =

New hairline drawn and confirmed with patient prior to incision

    =

Donor area trimmed and prepared in occipital region

    =

Local anesthesia administered to donor and recipient areas

    =

Individual follicular units extracted using motorized punch

    =

Grafts processed and stored in chilled holding solution throughout procedure

    =

Recipient sites created at precise angles to match natural hair direction

    =

Single hair grafts placed along the hairline for natural irregularity

    =

Multi-hair grafts placed posterior to the hairline for density

    =

Post-procedure dressing applied; discharge instructions provided

    POST OPERATIVE CARE AND RECOVERY

The recovery follows a predictable cycle. The first few days involve localised swelling and small scabs around each graft. By day five to ten, swelling resolves and most patients return to work. The transplanted hair enters a shedding phase between weeks two and four; this is expected and does not indicate graft failure. A dormant phase follows, typically lasting through months two and three, before new growth begins.

For a 4800-graft case, results develop progressively. Visible coverage typically becomes apparent from month four. Full density and hair maturation continue through the twelve to eighteen month window.

    RECOVERY TIMELINE

    TIMEFRAME WHAT TO EXPECT
Day 1 to 3 Localised swelling, redness, small scabs forming around grafts. Rest recommended.
Day 5 to 10 Swelling resolves. Scabs shed naturally. Most patients return to work.
Week 2 to 4 Shock loss phase; transplanted hair sheds. Normal and expected.
Month 2 to 3 Dormant phase. Follicles establishing blood supply. Minimal visible change.
Month 4 to 6 New growth begins. Fine hair emerging across the recipient area.
Month 8 to 12 Significant coverage visible. Hair thickening.
Month 12 to 18 Final result. Full density and texture maturation complete.

    CLINICAL OUTCOME

The patient tolerated the procedure well. No intraoperative or postoperative complications were recorded. Graft harvesting and placement were completed in a single session. The patient was discharged the same day.

Progressive results are expected over the twelve to eighteen month growth window, consistent with standard FUE hair transplant outcomes.
