Nasolabial folds deepen for two different reasons: volume loss and tissue descent. The right treatment depends on which one is driving the change. This blog covers what each treatment option actually does, when fillers work, when they do not, and when a facelift is the clearer answer.
It depends on why they are there. The cause determines the treatment.
Nasolabial folds are the lines that run from the sides of the nose to the corners of the mouth. Every face has them. The question is not whether they exist but whether they have deepened to the point where they are the primary concern. When they have, the treatment that works depends entirely on whether the folds are driven by volume loss, tissue descent, or both.
According to Dr Harikiran Chekuri, one of India’s pioneering plastic surgeons, “Nasolabial folds are one of the most commonly overtreated areas in aesthetic medicine. Filling the fold directly is the wrong approach in most cases. The fold is a symptom. The cause is cheek volume loss or mid-face descent, and treating the cause produces a far more natural result than filling the line.”
Why Do Nasolabial Folds Deepen?
Two causes. Different treatments for each.
- Volume loss: Facial fat diminishes, the cheek loses its support structure, and the skin descends. Dermal filler placed in the cheek, not the fold, is the right response
- Tissue descent: The mid-face SMAS layer drops with age, pushing tissue downward. Structural repositioning is required, not filling
- Skin laxity: Stretched skin loses its ability to remain taut. Skin tightening or facial surgery is needed depending on the degree of looseness
- Genetic structure: Some patients have deep folds from early adulthood regardless of ageing. The approach here is different from age-related deepening and is confirmed at assessment
How Each Treatment Works and When to Use It
The answer depends on which of the above is driving the fold.
Treatment | Best For | How It Works | Duration |
Cheek Filler (Hyaluronic Acid) | Volume loss, early deepening | Restores support above the fold, lifts tissue naturally | 9 to 18 months |
Cheek Filler (Calcium Hydroxylapatite) | Deeper volume deficit | Thicker structural correction, stimulates collagen | 18 to 24 months |
Fat Transfer | Patients wanting longer-lasting natural result | Own fat harvested and injected into mid-face | Permanent for integrated fat |
Thread Lift | Mild to moderate tissue descent | Threads physically reposition mid-face tissue upward | 12 to 18 months |
HIFU | Early skin laxity, mild descent | Targets SMAS layer, stimulates gradual collagen remodelling | 6 to 12 months |
Facelift Surgery | Significant tissue descent, deep established folds | SMAS repositioning lifts descended tissue structurally | 8 to 12 years |
What fillers cannot do: Fillers do not lift descended tissue. If the fold is deepened primarily by mid-face descent, filler adds bulk without correcting position. The result looks full but not naturally youthful.

Frequently Asked Questions
Does filler fix nasolabial folds permanently?
No. Hyaluronic acid filler lasts 9 to 18 months. Calcium hydroxylapatite lasts 18 to 24 months. Fat transfer produces longer-lasting results but requires a harvesting procedure. No filler is permanent and all require maintenance.
Where should filler be placed for nasolabial folds?
In the cheek, not directly in the fold. Filling the fold directly creates an unnatural appearance. Restoring cheek volume above the fold lifts the tissue and reduces the line more naturally.
Can a facelift fix nasolabial folds?
Yes, for folds caused by mid-face descent. A facelift repositions the descended tissue upward, directly reducing fold depth. It is the only procedure that addresses the structural cause rather than adding volume above it.
How long does thread lift last for nasolabial folds?
Approximately 12 to 18 months. The threads absorb over time and collagen forms around them, providing some continued support after the threads are gone. The result is less dramatic than a facelift but requires no surgical downtime.
Is HIFU effective for nasolabial folds?
HIFU is effective for mild to moderate laxity where early tightening of the SMAS layer reduces fold depth gradually. It is not effective for significant tissue descent or deep folds caused primarily by volume loss.
