Fractional Laser Skin Rejuvenation for Pigmentation and Texture
A 35-year-old woman with dull skin, uneven tone, and facial pigmentation underwent fractional laser skin rejuvenation by Dr. Harikiran Chekuri at Redefine Hair Transplant and Plastic Surgery Centre, achieving brighter, smoother, and more even-toned skin with minimal downtime and no complications.
PATIENT PROFILE
THE PROBLEM
She hadn’t stopped caring for her skin. That was the frustrating part.
Pooja had a skincare routine. She used SPF. She was consistent. But somewhere in her early thirties, the skin she had in her twenties started to drift. It looked flatter. Darker in patches. Her forehead and cheeks had pigmentation that hadn’t been there before. The texture felt rough in a way no serum was fixing.
She was not looking for anything dramatic. She worked in HR, she was in front of people all day, and she had started relying on makeup in a way she didn’t want to. The goal was to feel comfortable in her own skin again, without a full face on.
On assessment: Fitzpatrick type IV skin. Mixed melasma and sun-induced pigmentation across the cheeks and forehead. Mild to moderate textural irregularity. Overall skin radiance reduced. No active acne or inflammatory skin condition. A good candidate for fractional laser with appropriate pre and post care.
CONSULTATION & TREATMENT PLAN
Assessment findings
- Fitzpatrick type IV skin with mixed pigmentation pattern across the forehead and cheeks
- Combination of melasma and sun-induced pigmentation contributing to uneven tone
- Skin texture irregular with enlarged pores and mild surface roughness
- Overall radiance reduced. Skin appearing dull at rest.
- No active inflammatory conditions. No contraindications to fractional laser.
• Patient goal confirmed: improved brightness and even tone, not a dramatic change
Why fractional laser
Fractional laser therapy works by delivering precise laser energy to thousands of tiny columns in the skin, leaving surrounding tissue intact. This triggers the skin’s repair process: collagen is produced, pigment breaks down, and the surface remodels over the weeks following treatment. It produces visible improvement in tone and texture without the extended downtime of ablative procedures.
For Pooja, the combination of pigmentation and texture concerns made fractional laser the right choice. Topical products can maintain the skin but rarely correct established pigmentation or textural change at this level. The laser treats the cause at depth rather than just managing the surface.
PROCEDURE DETAILS
Step by step
- Baseline photographs taken before the session in standardised lighting
- Skin cleansed and makeup removed
- Topical numbing cream applied and left for 30 to 45 minutes
- Laser parameters set based on skin type and treatment area
- Fractional laser delivered across the full face in passes
- Pigmented areas treated with additional targeted passes
- Cooling applied to the skin after the session
- SPF and soothing moisturiser applied before discharge
- Post treatment instructions reviewed with Pooja before she left
POST TREATMENT CARE
Post treatment instructions were reviewed in detail before discharge and again at the follow up appointment.
Sun protection
- Apply broad spectrum sunscreen SPF 50 or above every 2 to 3 hours throughout the day
- Avoid direct sun exposure wherever possible for at least 2 weeks after treatment
- Wear a hat or use an umbrella when outdoors
- Sun protection is not optional after laser. Unprotected sun exposure can cause post-inflammatory pigmentation and undo the result.
Immediate post-treatment skin response
- Mild redness, warmth, and skin sensitivity for 2 to 5 days is expected and normal
- This feels similar to mild sunburn and settles on its own
- Avoid touching, rubbing, or scratching the skin during this period
- If peeling or flaking occurs, do not pick at it. Let the skin shed naturally.
Skincare during recovery
- Use a gentle cleanser only. Nothing active, abrasive, or foaming.
- Apply a hydrating, fragrance-free moisturiser to keep the skin calm
- Avoid makeup for at least 3 to 5 days or until redness has settled
- No exfoliants, retinoids, AHAs, BHAs, or vitamin C products for at least 5 days post treatment
- Follow the prescribed skincare routine strictly as the skin is more sensitive post laser
Heat and activity
- Avoid hot showers, saunas, steam rooms, and intense workouts for 48 to 72 hours
- Avoid waxing, facials, or any other skin treatment for at least 1 to 2 weeks
- Keep the skin cool and calm during the recovery period
Hydration and healing support
- Drink adequate water throughout the day to support skin healing from within
- Use soothing products recommended by the clinic if the skin feels particularly reactive

Frequently Asked Questions
What does fractional laser actually do to the skin?
Fractional laser delivers energy to thousands of microscopic columns in the skin, leaving the areas between untouched. This stimulates the skin’s natural repair response: collagen production increases, the damaged upper layers shed and are replaced by healthier skin, and pigment breaks down over time. The result is improved texture, tone, and radiance. It is more targeted than traditional resurfacing and requires less recovery time.
Is fractional laser suitable for darker skin tones?
It can be used safely on darker skin tones with the right parameters, timing, and pre-treatment preparation. The main risk with darker skin is post-inflammatory hyperpigmentation, where the skin produces excess pigment in response to the laser energy. This is manageable when the procedure is done by an experienced practitioner who sets the device appropriately and ensures the patient follows sun protection instructions rigorously post treatment.
How many sessions will I need?
This depends on the severity of the concern being treated. Mild pigmentation and early textural changes may show significant improvement after 2 to 3 sessions. More established pigmentation or deeper textural concerns typically need 4 to 6 sessions spaced 4 to 6 weeks apart. The treating doctor will advise a session plan based on the initial assessment and how the skin responds to the first treatment.
Will the pigmentation come back?
Laser clears existing pigmentation. Whether it returns depends largely on what caused it in the first place and whether those factors are controlled. Sun exposure without protection is the most common trigger for pigmentation recurrence. Patients who maintain consistent SPF use and follow the recommended skincare routine after their sessions tend to hold their results well. Maintenance sessions once or twice a year can also help sustain the improvement.
How quickly will I see a difference?
Some patients notice a brighter quality to their skin within a week of the first session, once the initial redness and peeling settle. But the real improvement builds over 3 to 4 weeks as the collagen remodelling process unfolds. The difference becomes more pronounced with each subsequent session. Most patients see a meaningful change in tone and texture by the end of their planned treatment course.
