Combined Body Contouring Surgery Case Study – Fat Grafting, Breast Reduction, Arm Lift & Tummy Tuck in Hyderabad

This case study highlights a combined body contouring surgery performed at Redefine Hair Transplant and Plastic Surgery Centre, Hyderabad. A 40-year-old patient underwent fat grafting, breast reduction, brachioplasty, and tummy tuck in a single session to address multiple areas of concern. The approach focused on achieving balanced body proportions, improved contour, and a streamlined recovery under expert surgical care.

THE PROBLEM

She came in knowing exactly what she wanted to fix.

Years of fluctuation in weight, combined with the natural effects of ageing, had left her body in a shape that no longer matched how she felt about herself. Her breasts had lost volume at the top and dropped. Her upper arms had excess skin that hung loose regardless of how much weight she lost or gained. Her abdomen was soft and slightly protruding, and the skin there had lost its firmness.

She was active, in reasonable health, and not looking for a subtle change. She wanted a proper reset. Four areas, addressed properly, in one go.

On examination: moderate breast ptosis with deflation of the upper pole. Bilateral arm skin laxity, worse on the medial surface. Abdominal skin laxity with a small fat deposit below the umbilicus. Donor fat availability assessed at the flanks and outer thighs.

She had no prior surgeries. Her goals were realistic and clearly communicated. The surgical team assessed her across two consultation sessions before proceeding.

CONSULTATION & TREATMENT PLAN

What was assessed

  • Breast shape, projection, and degree of ptosis
  • Upper arm skin laxity and excess tissue volume
  • Abdominal skin redundancy and muscle wall integrity
  • Donor fat sites for grafting and fat volume available
  • Overall fitness and suitability for combined general anaesthesia
  • Patient goals and recovery expectations discussed in detail

WHY A COMBINED SESSION

Running all four procedures in one session made sense for this patient. Her areas of concern were connected, the recovery would broadly overlap, and combining it into one anaesthesia event rather than four separate ones reduced her overall surgical risk and total recovery time.

This is not always the right call. But for Vinuthna, the assessment showed she was a good candidate for a combined session, and the surgical team was confident they could manage it safely.

PROCEDURE 01   |  Fat Grafting

Fat was harvested from the donor areas, processed, and transferred to improve volume and contour at the target sites. This technique uses the patient’s own tissue, which means no foreign material and a more natural feel to the result.

  • Donor fat harvested from flanks and outer thighs
  • Processed and prepared for reinjection
  • Injected in small increments at multiple depths for even distribution
  • Volume assessed after placement for symmetry

PROCEDURE 02   |  Breast Reduction

The breasts were reshaped and reduced in size to improve proportion and relieve the physical discomfort that comes with larger, ptotic breasts. Excess glandular tissue and skin were removed, and the nipple was repositioned to a more natural height.

  • Reduction in breast volume to achieve better proportionality
  • Excess skin removed and breast reshaped
  • Nipple repositioned using a standard technique appropriate for the degree of ptosis
  • Suture closure with scar management planned for post recovery

PROCEDURE 03  | Brachioplasty (Arm Lift)

Excess skin was removed from the inner upper arm. This is an area where no amount of exercise changes the loose skin once it is there. The incision runs along the inner arm and is placed to be as discreet as possible in its final healed position.

  • Excess skin and a small amount of underlying fat removed bilaterally
  • Incision placed along the medial arm for better concealment
  • Sutures placed in layers to reduce tension on the skin closure
  • Compression sleeves applied immediately post surgery

PROCEDURE 04  | Tummy Tuck (Abdominoplasty)

The lower abdominal skin and fat were addressed. The muscle wall was tightened at the midline, the excess skin below the umbilicus was removed, and the belly button was repositioned. A low horizontal scar sits along the bikini line in the healed result.

  • Lower abdominal skin removed through a horizontal incision above the pubic area
  • Rectus abdominis muscle plication to tighten the abdominal wall
  • Umbilicoplasty to reposition the belly button naturally
  • Surgical drain placed and managed per protocol
  • Abdominal binder fitted before leaving theatre

  • Avoid smoking and alcohol during the healing period
  • Stay well hydrated and eat well throughout recovery
  • Light activity from day 5 to 7. No strenuous exercise for 4 to 6 weeks.

  • Sleep on the back with the upper body slightly elevated
  • Keep incision areas clean and dry at all times
  • Avoid overextending the arms or making sudden movements
  • Complete all prescribed medications and antibiotics
  • Scar care begins only after the wounds have fully healed and the team gives clearance
  • Report any redness, discharge, fever, or unusual swelling immediately

Brachioplasty (Arm Lift)  —  Recovery Instructions

  • Wear compression sleeves for 4 to 6 weeks
  • Keep the arms elevated when resting to reduce swelling
  • Avoid lifting, pushing, and pulling activities
  • Limit arm movement during the early recovery days
  • Keep incision sites clean and dry
  • No strenuous exercise for at least 4 to 6 weeks

  • Report any excessive swelling, numbness, or discharge

Tummy Tuck  —  Recovery Instructions

  • Wear the abdominal binder continuously for about 6 weeks

Frequently Asked Questions

Is it safe to combine all four procedures in one session?

It depends on the patient. For someone who is in good health, has realistic goals, and has been assessed properly across multiple consultations, a combined session is a reasonable option. It reduces total anaesthesia exposure and allows a single consolidated recovery. For this patient, Dr. Harikiran and Dr. Farook assessed her carefully before proceeding and were satisfied she was a good candidate.

How long does recovery take after a combined body contouring surgery?

The first two weeks are the most restrictive. Most patients are back to light activity within 5 to 7 days of the non abdominal procedures and within 2 to 3 weeks for the tummy tuck. Strenuous activity waits until 6 to 8 weeks, depending on the area. The final result takes several months to show once all swelling has settled.

Will the scars from these procedures be visible?

All four procedures leave scars. The breast reduction scar follows the standard anchor or vertical pattern depending on the technique used. The arm lift scar runs along the inner arm. The tummy tuck scar sits low along the bikini line. Scars fade significantly over 12 to 18 months with proper care. The surgical team will advise on scar management as healing progresses.

How long do the results from fat grafting last?

Not all of the transferred fat survives. Typically 60 to 80 percent of grafted fat integrates and becomes permanent. The fat that does survive behaves like native tissue. It will respond to weight changes as the body normally does. Maintaining a stable weight after surgery helps preserve the result.
